区域性颈淋巴结清扫术治疗不同分期颈部淋巴结结核的疗效及安全性
Efficacy and safety of regional cervical lymph node dissection for cervical lymph node tuberculosis at different stages

Abstract
Objective To investigate the efficacy and safety of regional cervical lymph node dissection for cervical lymph node tu-berculosis at different stages,providing references for optimizing treatment strategies for this population.Methods A total of 150 cervical lymph node tuberculosis patients diagnosed and treated with regional cervical lymph node dissection at Public Health Clinical Center of Chengdu(June 2021-June 2024)were staged into:Stage Ⅰ(nodular type,n=22),Stage Ⅱ(infil-trative type,n=31),Stage Ⅲ(abscess type,n=46),and Stage Ⅳ(ulcerative type,n=51).Intraoperative blood loss,postoperative length of hospital stay,wound healing time,scar length,primary healing rate,benign scar healing rate,6-month focal recurrence rate,and overall incidence of surgery-related complications were compared.Results No significant differences were noted in intraoperative blood loss or postoperative length of hospital stay across stages(P＞0.05).Incision length,wound healing time,and scar length increased significantly with advancing stages(P＜0.05).Significant intergroup differ-ences were found in primary healing rate,benign scar healing rate,and 6-month recurrence rate(P＜0.05).Stage Ⅳ showed significantly lower primary healing and benign scar rates but higher focal recurrence than Stages Ⅰ-Ⅲ(P＜0.05).Antituber-culosis treatment efficacy showed no intergroup difference(P＞0.05).Incidence of surgery-related complications differed sig-nificantly(P＜0.05),with Stage Ⅳ having a higher incidence than Stages Ⅰ-Ⅲ(P＜0.05).Conclusion Regional cervical lymph node dissection is effective for cervical lymph node tuberculosis,but advanced stages correlate with prolonged healing,reduced primary healing,and increased focal recurrence/complications.Early surgical intervention is hence recommended for optimal outcomes.关键词
