以预防为导向的"专病小组"护理模式在ICU重症肺炎患者护理中的应用效果
Effectiveness of a prevention-oriented"special-disease team"nursing model in the care of ICU patients with severe pneumonia

Abstract
Objective To evaluate the effect of a prevention-oriented"special-disease team"nursing model on muscle strength status in intensive care unit(ICU)patients with severe pneumonia,with the aim of providing a reference for clinical care.Methods From May 2022 to May 2024,104 patients with severe pneumonia admitted to the ICU of Wuhan No.1 Hospital were enrolled.Patients were assigned,using a random number table,to a control group(n=52)receiving routine nursing and an observation group(n=52)receiving the prevention-oriented"special-disease team"nursing in addition to routine care.The British Medical Research Council muscle strength score(MRC-score),the modified Barthel Index,and the Pittsburgh Sleep Quality Index(PSQI)were compared between groups before intervention(at ICU admission)and after intervention(at ICU discharge).Duration of mechanical ventilation,length of ICU stay,and incidence of complications during ICU stay were also compared.Results Before intervention,there were no significant differences between the two groups in MRC-score or PSQI(P＞0.05).After intervention,both groups showed increased MRC-score and decreased PSQI compared with before interven-tion;moreover,the observation group had significantly higher MRC-score and lower PSQI than the control group(P＜0.05).Distribution of activities of daily living(self-care)levels did not differ between groups before intervention(P＞0.05),whereas after intervention,the distribution of self-care levels differed significantly between groups(P＜0.05).The observation group's duration of mechanical ventilation and length of ICU stay were significantly shorter than those of the control group(P＜0.05).The incidence of complications in the observation group was significantly lower than in the control group(P＜0.05).Conclusion For ICU patients with severe pneumonia,a prevention-oriented"special-disease team"nursing model can effectively promote muscle strength and sleep quality,strengthen activities of daily living,reduce the incidence of ICU-acquired weakness(ICU-AW),and shorten the length of ICU stay,with high safety profiles,which hold promise for wider clinical promotion.关键词
