当归四逆汤联合针刺治疗糖尿病周围神经病变临床观察
Clinical Observation of Chinese Angelica Frigid Extremities Decoction Combined with Acupuncture in Treating Diabetic Peripheral Neuropathy

Abstract
Objective:To observe the clinical efficacy of Chinese Angelica Frigid Extremities Decoction combined with acupuncture on di-abetic peripheral neuropathy(DPN).Methods:According to the random number table method,a total of 144 DPN patients treated at Kaifeng Hospital of Traditional Chinese Medicine from January 2019 to January 2022 were divided into the acupuncture group,the Chi-nese medicinal group,and the combined acupuncture-medicine group,with 48 cases in each group.All the three groups received con-ventional treatment first.Then the acupuncture group additionally received acupuncture treatment.The combined acupuncture-medi-cine group received Chinese Angelica Frigid Extremities Decoction in addition to the acupuncture treatment given to the acupuncture group.The Chinese medicinal group received Chinese Angelica Frigid Extremities Decoction.The changes in nerve conduction velocity[including sensory nerve conduction velocity(SNCV)and motor nerve conduction velocity(MNCV)of the median nerve and common peroneal nerve],blood glucose levels[including glycosylated hemoglobin(HbA1c),fasting blood glucose(FBG),2 hours postprandial blood glucose(2 hPG)],Toronto clinical scoring system(TCSS)scores,and TCM syndrome scores before and after treatment,clinical efficacy,and the occurrence of adverse reactions of the three groups were all observed.Results:After treatment,the SNCV and MNCV of the median nerve and common peroneal nerve in the combined acupuncture-medicine group were higher than those in the acupuncture group and the Chinese medicinal group,and all the differences were statistically significant(P＜0.05).The levels of HbA1 c,FBG,and 2 hPG in the combined acupuncture-medicine group were lower than those in the acupuncture group and the Chinese medicinal group,and all the differences were statistically significant(P＜0.05).The TCSS scores and TCM syndrome scores in the combined acupunc-ture-medicine group were lower than those in the acupuncture group and the Chinese medicinal group,and all the differences were sta-tistically significant(P＜0.05).The effective rate was 93.48％in the combined acupuncture-medicine group,75.56％in the acu-puncture group,and 77.78％in the Chinese medicinal group.The effective rate of the combined acupuncture-medicine group was higher than that of the acupuncture group and the Chinese medicine group,and the difference was statistically significant(P＜0.05).The incidence of adverse reactions was 10.87％in the combined acupuncture-medicine group,15.56％in the acupuncture group,and 15.56％in the Chinese medicinal group.The incidence of adverse reactions in the combined acupuncture-medicine group was lower than that in the acupuncture group and the Chinese medicinal group,but the difference among the three groups was not statistically sig-nificant(P＞0.05).Conclusion:Chinese Angelica Frigid Extremities Decoction combined with acupuncture can improve the neurologi-cal function,stabilize blood glucose levels,alleviate clinical symptoms,and enhance clinical efficacy in patients with DPN.关键词
