Impact of the completeness evaluation of complete mesocolic excision on patient prognosis in robot-assisted radical resection for colon cancer(with surgical video)

Abstract
Objective:To investigate the impact of completeness of complete mesocolic excision(CME)on the prognosis of colon cancer patients.Methods:102 patients who underwent robot-assisted radical resection for colon cancer at Jiangsu Province Hospital of Chinese Medicine from January 2020 to December 2022 were selected as the study subjects.Based on postoperative pathology and intraoperative assessment,they were divided into the CME complete group(n=68)and the CME incomplete group(n=34).The two groups were compared in terms of surgical indicators,perioperative complications,postoperative pathological characteristics,local recurrence rate,distant metastasis rate,3 year overall survival(OS),and disease-free survival(DFS).Postoperative follow up and prognostic analysis were conducted.Results:The CME complete group had a significantly higher number of dissected lymph nodes,3 year OS,and DFS compared to the CME incomplete group(P<0.05),with significantly lower local recurrence and distant metastasis rates(P<0.05).No significant differences were observed in operative time,intraoperative blood loss,resected bowel length,postoperative length of hospital stay,perioperative complication rates,or postoperative pathological features between the two groups(P>0.05).Kaplan Meier survival curve analysis showed that the 3 year OS and DFS in the CME complete group were significantly higher than those in the CME incomplete group.Multivariate Cox regression analysis indicated that the completeness of CME was an independent risk factor affecting patient prognosis(HR=2.45,95%CI:1.38～4.32,P=0.002).Conclusion:The assessment of CME completeness in robot-assisted radical resection for colon cancer has positive implications for local tumor control,prevention of distant metastasis,and long term survival.关键词
