Comparison of short-term clinical outcomes between robot-assisted and laparoscopic complete mesocolic excision for right-sided colon cancer via the caudodorsal-first combined with cephalic approach(with surgical video)

Abstract
Objective:To compare the short-term efficacy of robot-assisted versus laparoscopic complete mesocolic excision for right-sided colon cancer via caudodorsal-first combined with cephalic approach.Methods:Clinical data of 100 right-sided colon cancer patients who underwent CME at the Affiliated Hospital of Xuzhou Medical University form August 2020 to December 2023 were retrospectively analyzed.Patients were divided into the robotic group(n=50,receiving Da Vinci robot-assisted surgery)and laparoscopic group(n=50,receiving conventional laparoscopic surgery).Perioperative indicators and postoperative pathological outcomes were compared between the two groups.Results:Compared to the laparoscopic group,the robotic group showed reduced intraoperative blood loss,earlier time to first flatus and first oral intake,and higher total number of lymph nodes dissected(P<0.05),but longer operative time and higher hospitalization costs(P<0.05).Comparison between the two groups showed no statistically significant differences in conversion to open surgery rate,postoperative complication rate,time to drainage tube removal,length of hospital stay,tumor type,maximum tumor diameter,number of positive lymph nodes,or postoperative pathological staging(P>0.05).Conclusion:Robot-assisted complete mesocolic excision via caudodorsal-first combined with cephalic approach has advantages in reducing intraoperative bleeding,enhancing recovery,and improving lymph node dissection,which is safe and effective.关键词
