Clinical efficacy of robot-assisted laparoscopic rectal cancer surgery based on Denonvilliers fascia anatomy

Abstract
Objective:To investigate the clinical efficacy of robot-assisted laparoscopic rectal cancer surgery based on Denonvilliers fascia anatomy.Methods:80 male patients with mid-low rectal cancer who were treated at the Department of Gastroenterology,Minda Hospital of Hubei Minzu University from January 2023 to January 2024 were selected and divided into the study group(n=40,receiving robot-assisted laparoscopic rectal cancer surgery)and the control group(n=40,receiving conventional laparoscopic rectal cancer surgery).Intraoperative indicators,short-term postoperative recovery indicators,complications,pain scores,oncological outcomes,urinary function,sexual function,and follow-up results were compared between the two groups.Results:Compared with the control group,the study group had less intraoperative blood loss,higher integrity rate of Denonvilliers fascia,lower incidence of autonomic nerve injury,earlier time to first flatus and oral intake,shorter length of hospital stay,lower Visual Analog Scale(VAS)scores at 12,24,36,and 48 hours postoperatively,lower circumferential resection margin(CRM)positivity rate,lower International Prostate Symptom Score(IPSS)at 3 months postoperatively,and lower International Index of Erectile Function-5(IIEF-5)score(P<0.05).However,there were no significant differences in operative time,complication rate,local recurrence rate,or distant metastasis rate between the two groups(P>0.05).Conclusion:Robot-assisted laparoscopic surgery is superior to conventional laparoscopy in protecting Denonvilliers fascia,postoperative functional recovery,and radical resection of tumor,which can effectively reduce the risk of urogenital dysfunction and improve patients'quality of life.However,its long-term oncological efficacy requires further validation.关键词
