Application effect of hospital-to-home management model guided by change theory on patients in the perioperative period of robot-assisted radical gastrectomy

Abstract
Objective:To explore the effect of the hospital-to-home(H2H)management model guided by change theory on patients in the perioperative period of robot-assisted radical gastrectomy.Methods:204 patients who underwent robot-assisted radical gastrectomy in the First Affiliated Hospital of Xi'an Jiaotong University from March 2020 to March 2024 were selected.After screening by inclusion and exclusion criteria,181 gastric cancer patients were finally included in this study.According to different nursing methods,they were randomly divided into the control group(n=90,receiving routine nursing)and the observation group(n=91,receiving H2H management model under the guidance of change theory).Postoperative recovery indicators,serum nutritional indicators,nutritional status,quality of life,and complication rates were compared between the two groups.Results:Compared with the control group,the observation group had significantly earlier bowel sound recovery time,first eating time,first anal exhaust time,first defecation time,and shorter postoperative length of hospital stay(P<0.05).After nursing intervention,the levels of albumin,prealbumin,and hemoglobin in the two groups were both increased significantly,and they were higher in the observation group than the control group(P<0.05).After nursing,the Prognostic Nutritional Index(PNI)increased significantly,and the Subjective Global Assessment(SGA)score decreased significantly in the two groups(P<0.05).Moreover,the PNI of the observation group was higher than that of the control group,and the SGA score was lower(P<0.05).After nursing intervention,the Quality of Life Index(QLI)score and the Functional Assessment of Cancer Therapy-General(FACT-G)score increased significantly in the two groups,and they were higher in the observation group than the control group(P<0.05).The total complication rate in the control group was higher than that in the observation group(P<0.05).Conclusion:The H2H management model guided by change theory can effectively promote the recovery of nutritional status,accelerate postoperative rehabilitation,improve quality of life,and significantly reduce the complication rate in patients during the perioperative period of robot-assisted radical gastrectomy.关键词
