Abstract
Objective To explore the efficacy of Wiltse approach combined with transforaminal lumbar interbody fu-sion(TLIF)in the treatment of far lateral lumbar disc herniation(FLLDH).Methods The 46 patients with FLL-DH involving both the exiting and traversing roots were treated with Wiltse approach combined with TLIF.The pain VAS of low back and leg pain,ODI,JOA score and symptom relief were recorded.The clinical efficacy was evalua-ted by the modified MacNab criteria.Results All patients were followed up for 3～24 months.The pain VAS of low back and leg pain,ODI and JOA score:They were better at 7 d,3 months and the last follow-up after surgery than before surgery(P＜0.05);and the scores were improved with the extension of postoperative time(P＜0.05).At the last follow-up,according to the MacNab evaluation criteria,the efficacy was excellent in 39 cases,good in 5 ca-ses,fair in 2 cases,and the excellent and good rate was 95.7％;the symptoms of low back pain and radiating pain of leg in patients were relieved,and 2 cases had mild sensory loss in the affected calf.Conclusions The Wiltse ap-proach combined with TLIF can effectively improve the clinical symptoms of FLLDH patients,which has high safety and satisfactory effect.关键词
