Minimally invasive transforaminal lumbar interbody fusion under microscope-assisted fixed channel in the treatment of lumbar degenerative diseases

Abstract
Objective To explore the efficacy of minimally invasive transforaminal lumbar interbody fusion(MIS-TLIF)under microscope-assisted fixation channel in the treatment of lumbar degenerative diseases.Methods The 48 patients with lumbar degenerative diseases were divided into the observation group(The 24 cases were treated with MIS-TLIF under microscope-assisted fixation channel)and the control group(The 24 cases were treated with MIS-TLIF under Quadrant channel)according to different surgical methods.The perioperative observation indicators,pain relief degree and lumbar function recovery were compared between the two groups.Results All patients were fol-lowed up for 12~18 months.The intraoperative blood loss,postoperative drainage volume in the observation group were better than those in the control group(P<0.05).At 1 week and 3 months after surgery,the VAS of lower back pain in the observation group were lower than those in the control group(P<0.05),and the VAS of lower extremity pain at 1 week after surgery in the observation group was lower than those in the control group(P<0.05).At 1 week,3 months and 12 months postoperation,the JOA scores after surgery in the observation group were higher than those in the control group(P<0.05).At the last follow-up,there was no statistical difference in the excellent and good rate of lumbar function evaluated by MacNab criteria between the two groups(P<0.05).Conclusions In the treatment of lumbar degenerative diseases,both MIS-TLIF under microscope-assisted fixation channel and Quadrant channel can achieve satisfactory clinical efficacy.MIS-TLIF under microscope-assisted fixation channel can provide precise surgical operation for the surgeon,and has obvious minimally invasive advantages in the early postoperative pe-riod,but the operation time is relatively longer.关键词
