Abstract
Objective To explore the efficacy of open reduction and Kirschner wire internal fixation with small inci-sion in the treatment of clavicle fracture in children.Methods Small incision open reduction and Kirschner wire in-ternal fixation were used to treat 37 children with clavicle fractures.The fracture reduction,fracture healing,occur-rence of postoperative complications and range of motion of the shoulder joint were recorded.The therapeutic effect was evaluated using the Bigliani shoulder joint function scoring standard.Results All patients were followed up for 6～12(7.3±0.9)months.All fractures were anatomically reduced.The fracture healing time was 2～4(3.1±0.6)months.Postoperative follow-up revealed that in one case,the Kirschner wire had migrated proximally and protruded from the clavicle at the postoperative 1-month examination.Another case showed slight bending of the Kirschner wire within the medullary cavity of the clavicle at the 6-week follow-up.The remaining patients exhibited no complications such as loosening or migration,bending of the Kirschner wire,skin penetration or exposure at the wire end.Addition-ally,there were no instances of bone resorption with sclerosis,fracture redisplacement or internal fixation failure.At the last follow-up,shoulder joint range of motion:Abduction was 75°～89°(78.1°±1.7°),adduction was 20°～35°(26.8°±1.4°),anterior flexion was 75°～85°(77.6°±1.1°),and posterior extension was 30°～40°(35.1°±0.4°),internal rotation was 50°～65°(56.7°±1.2°),external rotation with 50°～60°(54.8°±0.3°).The thera-peutic effect was evaluated according to the Bigliani shoulder joint function scoring standard:The 35 cases were excel-lent and 2 cases were good.Conclusions For the treatment of clavicle fracture in children,small incision open re-duction and Kirschner wire internal fixation is simple to operate,reliable in fixation,convenient to remove the internal fixator,and has few complications and good therapeutic effect.关键词
