巩膜扣带术治疗孔源性视网膜脱离的临床疗效观察
Clinical efficacy of scleral buckling surgery in the treatment of rhegmatogenous retinal detachment

Abstract
Objective With the widespread promotion of vitrectomy,the utilization rate of scleral buckling sur-gery in the treatment of rhegmatogenous retinal detachment has significantly declined.This study aims to re-evaluate the clinical value of traditional scleral buckling surgery.Methods A retrospective analysis was conducted on 147 patients(150 eyes)with rhegmatogenous retinal detachment treated at Tongji hospital from January 2023 to December 2024.All cases underwent scleral buckling surgery,with postoperative follow-up mean lasting 3 months.Results The study in-cluded 147 patients(150 eyes),comprising 74 males(76 eyes)and 73 females(74 eyes).Unilateral involvement was ob-served in 144 cases,while bilateral in 3 cases.The mean patient age was 39±17 years.All eyes had proliferative vitreo-retinopathy(PVR)graded≤C1.Single retinal breaks were present in 56 eyes,and multiple breaks in 94 eyes.Retinal de-tachment was limited to 1～2 quadrants in 139 eyes,and macular involvement was noted in 91 eyes.Retinal breaks were located anterior to the equator in 138 eyes.Surgical procedures included simple scleral buckling(142 eyes),combined scleral buckling and encircling band(7 eyes),and simple encircling band(1 eye).Subretinal fluid drainage was per-formed in 8 eyes.The mean postoperative follow-up duration was 3 months.Anatomical success(retinal reattachment)was achieved in 98%of cases(147/150),and 94%of eyes(141/150)maintained or improved best-corrected visual acuity postoperatively.No serious ocular complications occurred.Conclusions Scleral buckling surgery remains a classic and foundational technique for treating rhegmatogenous retinal detachment.It demonstrates high success rates in anatomical re-attachment and favorable visual outcomes for non-complex rhegmatogenous retinal detachment.As a time-tested approach,it should be preserved as a complementary option to vitrectomy.Standardized training for young ophthalmologists is crucial to ensure mastery of this technique.关键词
