基于"热伏营血,瘀滞孙络"探讨糖尿病足溃疡的治疗思路
Exploring treatment approach for diabetic foot ulcer based on the theory of"heat entering nutrient blood and stasis obstructing tertiary collaterals"

Abstract
Diabetic foot ulcer(DFU)significantly impacts patients'quality of life.Based on the theory of"heat entering nutrient blood and stasis obstructing tertiary collaterals",the review explores the feasibility and rationale for DFU treatment.It proposes that DFU primarily stems from inherent deficiency(ben xu),characterized by constitutional Yin deficiency and insufficiency of the spleen and kidney.Concurrently,it features manifest excess(biao shi)with the pathogenesis of heat entering nutrient blood and stasis obstructing tertiary collaterals.This combination of root deficiency and manifest excess,where stasis and heat intertwine,leads to multiple complications and renders DFU refractory to healing.Treatment should address both the root and the manifestations.The proposed principles involve tonifying the spleen and kidney,boosting Qi,and nourishing Yin to treat the root(ben),combined with clearing heat,cooling blood,dissipating stasis,and unblocking collaterals to treat the manifestations(biao),forming the"tonifying,clearing and dispersing"formula strategy to offer innovative perspectives for clinical DFU management.This forms the formula design strategy of"tonifying,clearing and dispersing",aiming to provide new perspectives for the clinical management of DFU.关键词
