针刺内关对广泛性焦虑症DKT曲线参数及心率变异性的影响
The impact of acupuncture at Neiguan(PC6)on DKT curve arameters and heart rate variability in generalized anxiety disorder

Abstract
Objective To observe changes in Korotkoff sound visualization sphygmomanometry(DKT)curve parameters before and after acupuncture at Neiguan(PC6)in patients with generalized anxiety disorder(GAD),and analyze correlations between DKT parameters and heart rate variability(HRV).Methods Sixty-six GAD patients were randomized into an acupuncture group(n=33)and a drug group(n=33),with 30 healthy subjects as a blank group.The acupuncture group received bilateral PC6 and non-acupoint stimulation(0.5 cun ulnar to the palmaris longus tendon)using 0.35mm×25mm insulated needles with dense-disperse wave electroacupuncture(4Hz/20Hz),20min/session,3sessions/week for 4 weeks.The drug group received lorazepam(0.5mg/day at bedtime)for 4 weeks.The blank group underwent single DKT measurement.DKT curves were recorded pre-and post-treatment in acupuncture/drug groups,with HRV frequency-domain parameters[Low-Frequency power(LF),High-Frequency power(HF),LF/HF ratio]measured in the acupuncture group.Comparisons included:①Intergroup differences in DKT changes;②Correlations between DKT parameters and HRV in the acupuncture group;③Baseline DKT differences between healthy subjects and GAD patients.Results GAD patients exhibited significantly higher baseline DKT parameters versus healthy subjects(P<0.05).Post-treatment,the acupuncture group showed significantly lower a_pri,d_pri,e_pri,and standard deviation than controls(P<0.05),with both groups demonstrating reduced DKT parameters versus baseline(P<0.05).HRV analysis revealed post-acupuncture reductions in LF(77.90<113.50,P=0.001)and LF/HF(0.47<1.15,P<0.001),and increased HF(140.40>82.80,P=0.034).Correlation analysis indicated,LF positively correlated with a_pri,c_pri,and peak amplitude(r>0,P<0.05),HF negatively correlated with a_pri,d_pri,and peak amplitude(r<0,P<0.05),LF/HF positively correlated with a_pri,c_pri,d_pri,and peak amplitude(r>0,P<0.05).Conclusion GAD patients exhibited significantly higher baseline DKT parameters.Acupuncture effectively modulates elevated DKT parameters and HRV dysregulation in GAD,with significant correlations between cardiovascular and autonomic outcomes.关键词
