超声引导下分段剥脱术联合腔内泡沫硬化治疗大隐静脉曲张的疗效观察
Efficacy of color Doppler ultrasound-guided segmental stripping combined with foam sclerotherapy in the treatment of great saphenous vein varicosity

Abstract
Objective To investigate the clinical efficacy and safety of color Doppler ultrasound-guided segmental stripping combined with foam sclerotherapy in the treatment of great saphenous vein varicosity.Methods A retrospec-tive analysis was conducted on 64 patients who underwent surgical treatment for great saphenous vein varicosity.According to the surgical method,32 patients in the observation group underwent high ligation and segmental stripping under color Doppler ultrasound guidance combined with foam sclerotherapy,while 32 patients in the control group underwent high liga-tion and stripping combined with punctate stripping of superficial varicose veins.Perioperative indicators(operation time,intraoperative blood loss,the number of incisions,and length of hospital stay),Venous Clinical Severity Score(VCSS),and Chronic Venous Insufficiency Quality of Life Questionnaire 20(CIVIQ-20)scores before and 3 months after surgery,the incidence of complications(wound infection,lymphatic leakage,subcutaneous hematoma,saphenous nerve injury)at 1 month postoperatively,and recurrence at 1 year postoperatively were recorded and compared between the two groups.Results The observation group had significantly shorter operation time,less intraoperative blood loss,fewer incisions,and shorter hospital stay than the control group(all P<0.05).There were no significant differences in preoperative VCSS and CIVIQ-20 scores between the two groups(both P>0.05).At 3 months postoperatively,both groups showed decreased VCSS scores and increased CIVIQ-20 scores compared with the preoperative values,with more significant changes in the observation group(both P<0.05).In the observation group,there were 0 case of wound infection,0 case of lymphatic leakage,1 case of subcutaneous hematoma,and 0 case of saphenous nerve injury,with a total complication rate of 3.1%;in the control group,there were 1,2,4,and 2 cases,respectively,with a total complication rate of 28.1%.The compli-cation rate was significantly lower in the observation group than in the control group(P<0.05).No recurrence was ob-served in either group at 1 year postoperatively.Conclusion Color Doppler ultrasound-guided segmental stripping com-bined with foam sclerotherapy for great saphenous vein varicosity can shorten operation and hospital stay,reduce intraoper-ative blood loss,and improve safety,with faster postoperative recovery and better quality of life.关键词
