针刺治疗卒中后口咽期吞咽障碍的疗效观察
Efficacy observation of acupuncture for post-stroke oropharyngeal dysphagia

Abstract
Objective To observe the clinical efficacy of acupuncture by the theory of three elements of heaven,earth and man in treating post-stroke oropharyngeal dysphagia.Method A total of 106 patients with post-stroke oropharyngeal dysphagia were randomly divided into a three elements acupuncture group and a conventional acupuncture group,with 53 cases in each group.Both groups received basic treatment and rehabilitation training.The conventional acupuncture group additionally received conventional acupuncture,while the three elements acupuncture group received acupuncture guided by the theory of three elements of heaven,earth and man,including scalp electroacupuncture at the element of heaven,fire needling at the element of man,and neck acupuncture at the element of earth.Changes in TCM syndrome scores(dysphagia,water drinking cough,tongue stiffness,and sluggish speech),water swallow test(WST)scores,standardized swallowing assessment(SSA)scores,and swallowing-quality of life(SWAL-QOL)scores were observed before and after treatment in both groups.Clinical efficacy,pulmonary infection rate,and gastric tube retention rate were compared between the two groups.Result After treatment,all individual TCM syndrome scores in both groups were lower than before treatment within the same group(P＜0.05).The three elements acupuncture group showed lower scores in dysphagia,water drinking cough,and tongue stiffness compared to the conventional acupuncture group(P＜0.05).After treatment,WST,SSA,and SWAL-QOL scores in both groups improved compared to before treatment within the same group(P＜0.05).The three elements acupuncture group had lower WST and SSA scores than the conventional acupuncture group(P＜0.05).The marked effectiveness rate in the three elements acupuncture group was higher than that in the conventional acupuncture group(P＜0.05),and the gastric tube retention rate was lower(P＜0.05).No significant difference was observed in the pulmonary infection rate between the two groups(P＜0.05).Conclusion On the basis of basic treatment and rehabilitation training,acupuncture by the theory of three elements of heaven,earth and man can significantly improve swallowing function in patients with post-stroke oropharyngeal dysphagia,with superior efficacy compared to the conventional acupuncture.关键词
