Abstract
Objective To compare the diagnostic accuracy of spectral CT and traditional multi-slice spiral CT for vulnerable carotid artery plaques.Methods Clinical diagnostic tests on CT detection of vulnerable carotid plaques were systematically searched in PubMed,Cochrane Library,Embase,Web of Science,CBM,CNKI,and Wanfang Data from inception to January 10,2025.The quality assessment of diagnostic accuracy studies-2(QUADAS-2)evaluation system in Review Manager 5.4 was applied to assess the risk of bias in the included literature,while Meta-Disc 1.4 and Stata 18.0 were used for statistical analysis.Results A total of 12 studies were included in this analysis,with 5 using the spectral CT and 7 using the conventional multi-slice CT.The pooled sensitivity of spectral CT and conventional multi-slice CT in the diagnosis of carotid artery vulnerable plaque were 0.92(95%CI=0.81-0.97)and 0.60(95%CI=0.31-0.83),respectively,and the pooled specificity were 0.92(95%CI=0.86-0.96),0.80(95%CI=0.51-0.94),respectively,and the pooled positive likelihood ratios were 12.25(95%CI=6.46-23.20)and 2.93(95%CI=0.79-10.86),respectively,and the pooled negative likelihood ratios were 0.08(95%CI=0.03-0.22),0.51(95%CI=0.22-1.18),and diagnostic odds ratios were 150.03(95%CI=39.19-574.40)and 5.76(95%CI=0.71-46.57),respectively.The area under the curve of receiver operating characteristic curves were 0.94(95%CI=0.92-0.96)and 0.77(95%CI=0.73-0.80),respectively.Conclusion The diagnostic value of spectral CT for carotid artery vulnerable plaques is higher than that of conventional multi-slice CT.关键词
