Evaluation on the Clinical Effect of Percutaneous Nephrolithotomy Combined with Flexible Ureteroscopy in the Treatment of>2 cm Renal Stones

Abstract
Objective To evaluate the clinical effect of percutaneous nephrolithotomy combined with flexible ureteroscopy in the treatment of>2 cm renal stones.Methods 82 patients with>2 cm renal stones were selected and divided into nephrolithotripsy group(41 cases)and combined surgery group(41 cases)according to the different treatment plans.The nephrolithotripsy group was treated with percutaneous nephrolithotomy,and the combined surgery group was treated with percutaneous nephrolithotomy combined with flexible ureteroscopy.The perioperative indicators,stone clearance rate,complications and renal function[urea nitrogen(BUN),blood creatinine(Scr)]were compared between the two groups.Results Compared with the nephrolithotripsy group,the combined surgery group had longer operation time,lower intraoperative blood loss,and shorter hospitalization days(P<0.05).The stone clearance rate of the combined surgery group was 95.12%,higher than 78.05%of the nephrolithotripsy group(P<0.05).1 month after surgery,the BUN and Scr levels of the combined surgery group were lower than those of the nephrolithotripsy group(P<0.05).The incidence of stone complications in the combined surgery group were 4.88%,lower than 19.51%in the nephrolithotripsy group(P<0.05).Conclusions Percutaneous nephrolithotomy combined with flexible ureteroscopy in the treatment of>2 cm renal stones has significant effect.Although the comprehensive treatment plan prolongs the operation time compared with the single surgical approach,it can significantly control the blood loss,reduce the body damage,increase the stone clearance rate,reduce the risks of postoperative bleeding and infection,and facilitate the postoperative rehabilitation of patients.关键词
