MRI迷路水神经成像对三叉神经痛患者神经血管压迫的评估价值
Evaluation Value of MRI Labyrinthine Neurohydrography on Neurovascular Compression in Patients with Trigeminal Neuralgia

Abstract
Objective To explore the evaluation value of magnetic resonance imaging(MRI)labyrinthine neurohydrography on neurovascular compression(NVC)in patients with trigeminal neuralgia(TN).Methods The clinical data of 104 patients with TN admitted to Anyang Sixth People's Hospital from January 2022 to January 2025 were retrospectively analyzed.All patients were examined by MRI labyrinthine neurohydrography before surgery,underwent microvascular decompression(MVD)surgery.The consistency of preoperative MRI determination with location of offending vessel,neurovascular compression degree and those seen during MVD surgery,as well as the consistency of preoperative MRI determination with direction of neurovascular compression and distribution area of TN symptoms were analyzed.The morphological parameters on the symptomatic side and non-symptomatic side of TN patients were compared.Results Kappa test showed that the location of offending vessels determined by preoperative MRI was highly consistent with the result seen during MVD surgery(Kappa value=0.796,P<0.001),the direction of neurovascular compression determined by preoperative MRI was highly consistent with the distribution area of TN symptoms(Kappa value=0.762,P<0.001),the degree of neurovascular compression determined by preoperative MRI was highly consistent with that determined by MVD surgery(Kappa value=0.871,P<0.001).The cross-sectional area,pontine angle and sectional area of cerebellopontine angle cistern on the symptomatic side of TN patients were smaller than those on the non-symptomatic side(P<0.05).Conclusions MRI labyrinthine neurohydrography can accurately identify the location,compression direction and compression degree of the offending vessels in TN patients.Moreover,there are morphological parameters changes such as trigeminal nerve atrophy,pontine angle reduction and cerebellopontine angle cistern stenosis on the symptomatic side of TN patients,which can provide an important reference for clinical assessment of NVC in TN patients.关键词
