超声引导下细针穿刺细胞学检查对甲状腺癌颈部淋巴结转移的诊断价值
Diagnostic Value of Ultrasound-Guided Fine Needle Aspiration Cytology Ex-amination for Cervical Lymph Node Metastasis of Thyroid Cancer

Abstract
Objective To explore the diagnostic value of ultrasound-guided fine needle aspiration cytology(US-FNAC)examination for cervical lymph node metastasis of thyroid cancer.Methods The clinical data of 112 patients with thyroid cancer confirmed by postoperative pathological diagnosis in Puyang People's Hospital from April 2023 to April 2025 were retrospectively analyzed,and all patients received preoperative high-frequency color Doppler ultrasound and US-FNAC examination.Taking postoperative pathological diagnosis as"gold standard",the consistency and diagnostic efficiency of the two methods for cervical lymph node metastasis of thyroid cancer were compared.Results By Kappa consistency test,high-frequency color Doppler ultrasound examination had moderate consistency with pathological diagnosis of cervical lymph node metastasis of thyroid cancer(Kappa value=0.722,P<0.001),and US-FNAC examination had high consistency with pathological diagnosis of cervical lymph node metastasis of thyroid cancer(Kappa value=0.944,P<0.001).The specificity(98.51%vs.88.06%),positive predictive value(97.73%vs.82.61%)and accuracy(97.32%vs.86.61%)of US-FNAC examination in the diagnosis of cervical lymph node metastasis of thyroid cancer were higher than those of high-frequency color Doppler ultrasound examination(P<0.05).Conclusions Compared with high-frequency color ultrasound examination,US-FNAC examination has higher efficacy in diagnosing cervical lymph node metastasis of thyroid cancer,and has high clinical diagnostic value for cervical lymph node metastasis of thyroid cancer.关键词
