基于螺旋断层放疗的模块化挽救方案在复发转移食管癌治疗中的应用研究
Study on the Application of Modular Rescue Scheme Based on Helical To-motherapy in the Treatment of Recurrent Metastatic Esophageal Cancer

Abstract
Objective To explore the application effect of modular rescue scheme based on helical tomotherapy(HT)in the treatment of recurrent metastatic esophageal cancer.Methods A total of 182 patients with recurrent metastatic esophageal cancer admitted to our hospital from August 2019 to January 2021 were randomly divided into observation group and control group,with 91 cases in each group.The observation group was treated with modular rescue scheme based on HT,and the control group was treated with conventional intensity-modulated radiation therapy(IMRT).The short-term efficacy,adverse reactions,survival rates,changes of quality of life and the levels of tumor markers were compared between the two groups.Results The ORR and DCR of the observation group were 68.13%and 94.51%,respectively,while the ORR and DCR of the control group were 45.05%and 78.02%,respectively,the ORR and DCR of the observation group were higher than those of the control group(P<0.05).The incidences of leukopenia,thrombocytopenia,anemia,nausea,vomiting,radiation esophagitis,and radiation pneumonia in the observation group were lower than those in the control group(P<0.05),moreover,there was no grade 3～4 severe radiation esophagitis in the observation group,whereas there were 5 cases of grade 3 radiation esophagitis in the control group.The 1-year,2-year and 3-year survival rates of the observation group were 89.01%,67.03%and 43.96%,respectively,while the control group was 72.53%,49.45%and 29.67%respectively,the 1-year,2-year and 3-year survival rates of the observation group were higher than those of the control group(P<0.05).After treatment,the quality of life scores of the two groups were higher than those before treatment,and the quality of life scores of the observation group after treatment were higher than those of the control group(P<0.05).After treatment,the levels of serum CEA and SCC in the two groups were lower than those before treatment,and the levels of serum CEA and SCC in the observation group were lower than those in the control group(P<0.05).Conclusions The modular rescue scheme based on HT for the treatment of recurrent metastatic esophageal cancer can significantly improve the short-term efficacy,reduce the incidence of adverse reactions,improve the quality of life,increase the survival rate,and lower the levels of tumor markers in patients.关键词
