乳腺钼靶联合DCE-MRI对乳腺炎和非肿块样强化乳腺癌的鉴别诊断价值
Differential Diagnosis Value of Mammography Combined with DCE-MRI on Mastitis and Non-Mass-Like Enhanced Breast Cancer

Abstract
Objective To explore the differential diagnosis value of mammography combined with dynamic contrast-enhanced magnetic resonance imaging(DCE-MRI)on mastitis and non-mass-like enhanced breast cancer.Methods A total of 81 female patients with non-mass-like enhanced breast lesions admitted to Xinyang Central Hospital for diagnosis and treatment from January 2022 to January 2025 were selected as the research objects.All patients underwent mammography and DCE-MRI examination.The pathological examination results were used as the"gold standard",to count the number of confirmed cases of mastitis and non-mass-like enhanced breast cancer,as well as the mammography and DCE-MRI imaging features of the two types of patients.The differential diagnostic efficacy of mammography and DCE-MRI alone and in combination for mastitis and non-mass-like enhanced breast cancer was also compared and analyzed.Results Pathological examination showed that 45 cases were diagnosed as non-mass-like enhanced breast cancer(55.56%)and 36 cases were diagnosed as mastitis(44.44%)among 81 patients.The proportion of burr-like edge,uneven enhancement,type ⅢTIC and Kaiser score in patients with non-mass-like enhanced breast cancer were higher than those in patients with mastitis(P<0.05).The Kappa values of mammography and DCE-MRI alone and their combination in the differential diagnosis of mastitis and non-mass-like enhanced breast cancer were 0.634,0.705 and 0.801,respectively,the results of combined examination and pathological examination were highly consistent.The sensitivity and accuracy of mammography combined with DCE-MRI in the differential diagnosis of mastitis and non-mass-like enhanced breast cancer were 93.33%and 97.22%,respectively,higher than those of mammography(73.33%,91.67%)and DCE-MRI(75.56%,86.11%)alone(P<0.05).Conclusions Compared with mammography and DCE-MRI examination alone,the combination of mammography and DCE-MRI is more effective in differential diagnosis of mastitis and non-mass-like enhanced breast cancer,which can be used for clinical reference.关键词
