Abstract
Objective To compare the differences in stress response, complications and survival prognosis after laparoscopic radical resection and open radical resection in colorectal cancer(CRC)patients with different body mass indexes(BMIs).Methods A total of 82 CRC patients admitted from January 2023 to June 2024 were divided into the laparoscopic group(treated with laparoscopic radical resection)and the open group(treated with open radical resection)according to different surgical methods, and were divided into the normal group(18.5 kg/m2≤BMI<25.0 kg/m2)and the overweight/obese group(≥25.0 kg/m2)according to BMI.The surgical indicators(operation time, intraoperative blood loss, intestinal function recovery time, length of hospital stay),stress response indicators[malondialdehyde(MDA),glutathione peroxidase(GSH-Px),superoxide dismutase(SOD)],postoperative complications and 6-month survival prognosis were com-pared among different groups. Results The intraoperative blood loss, intestinal function recovery time and length of hospital stay in the laparoscopic group were less or shorter than those in the open group(P<0.05),while there was no significant difference between the overweight/obese group and the normal group(P>0.05).There was no significant difference in the operation time between the laparoscopic group and the open group(P>0.05),while the operation time in the overweight/obese group was longer than that in the normal group(P<0.05).Compared with the laparoscopic group, the levels of GSH-Px and SOD in the open group decreased after surgery, and the decrease was more significant in the overweight/obese group(P<0.05);while the level of MDA in the open group increased after surgery, and the increase was more significant in the overweight/obese group(P<0.05).There was no significant difference in the total incidence of postoperative complications and 6-month survival rate between the laparoscopic group and the open group, and between the overweight/obese group and the normal group(P>0.05).Conclusion Laparoscopic radical resection can more effectively reduce the postoperative oxidative stress response than open surgery, and at the same time,it has more advantages in perioperative safety. The operation time of obese patients is relatively prolonged, and the postoperative oxidative stress response is more obvious.关键词
