Abstract
AIM To evaluate the efficacy of a pre-audit system in alerting clinicians to prohibited drugs for hospitalized patients with renal insufficiency,thereby providing guidance for promoting rational clinical prescribing practices in this patient population.METHODS A total of 201 836 inpatient medical orders from the pre-audit system between January and June 2024 were selected.Level 6 warning alerts related to the use of contraindicated drugs in hospitalized patients with renal insufficiency were screened.Data on the patients' admitting departments,gender,age,diagnoses,medical order numbers,order details,medication usage,and warning alerts were collected for retrospective analysis.RESULTS Among the 201 836 inpatient medical orders,there were 555 level 6 alert medical orders for renal insufficiency.Electrolyte and nutritional drugs triggered the highest proportion of level 6 alerts(401 orders,accounting for 72.3％).Statistically significant differences were observed in the distribution of level 6 warning orders among inpatients with renal insufficiency across different departments(P＜0.05),with internal medicine departments exhibiting a higher proportion of such alerts.In addition,based on the latest drug labels and evidence-based medical evidence,the prescribing review rules for endogenous creatinine clearance rate and estimated glomerular filtration rate were optimized for 7 drugs within the pre-audit system.CONCLUSION Prescriptions for electrolyte and nutritional medications are more likely to trigger level 6 alerts for hospitalized patients with renal insufficiency,with such alerts predominantly occurring in internal medicine departments.The use of the prescription review system has significantly improved the ability to provide early warning of prohibited drug prescriptions for patients with renal insufficiency,ensuring the safety and rationality of clinical drug use.关键词
