Application of Hierarchical Nursing Management Based on the Intensive Care Nursing Scoring System in Patients with Stanford Type A Aortic Dissection

Abstract
Objective To evaluate the effect of hierarchical nursing management constructed based on the intensive care nursing scoring system(ICNSS)on the perioperative nursing outcomes of patients with Stanford type A aortic dissection.Methods A total of 366 surgical patients with Stanford type A aortic dissection at the Department of Cardiothoracic Surgery of the Eastern Theater Command General Hospital from June 2023 to June 2025 were selected as research subjects.They were ran-domly divided(using a random number table method)into an experimental group(n=183,receiving ICNSS-based hierarchical nursing)and a control group(n=183,receiving routine nursing).The study observed changes in blood pressure levels(including systolic and diastolic pressures)and Brief Health Interview(SF-36)scores before and after intervention,while also analyzing complication rates,hospital stays,and nursing satisfaction scores.Results Before the intervention,there was no significant difference in blood pressure levels between groups(P>0.05).After intervention,both groups showed lower systolic and diastolic pressures than pre-intervention levels(P<0.05),with the experimental group achieving significantly lower values than the control group(P<0.05).SF-36 scores showed no statistically significant differences before intervention(P>0.05).Post-intervention,SF-36 scores increased across all dimensions compared to pre-intervention levels(P<0.05).The experimental group demonstrated a significantly lower postoperative complication rate(12.57%vs.30.05%,P<0.05),the hospitalization time(9.2±1.5 d)was significantly shorter than that of the control group(14.3±2.1 d),and a marked improvement in nursing satisfaction scores(98.4%vs.90.2%,P<0.05)compared to the control group.Conclusion ICNSS-based hierarchical nursing can effectively improve the clinical outcomes for surgical patients with Stanford type A aortic dissection and optimize the allocation of nursing resources.关键词
