Clinical challenges and research advances in radiotherapy-induced xerostomia
摘要
Abstract
Radiotherapy serves as a primary or adjuvant treatment for head and neck cancer.However,while it effectively eradicates cancer cells,it also poses a risk of damaging normal tissues.Radiotherapy not only kills tumor cells but also causes damage to normal tissues.Xerostomia,one of the most prevalent late-stage complications,severely impacts patients'quality of life.Contemporary clinical management encounters multifaceted challenges:at the diagnostic level,current tools that rely on subjective rating scales exhibit limited capacity to differentiate functional xerostomia from neurological sensory disorders;in therapeutic interventions,physical therapy protocols are undermined by poorly defined operational parameters,leading to suboptimal patient adherence;the development of personalized predictive models is obstructed by non-linear relationships between dosimetric parameters and genomic susceptibility;and regenerative medicine approaches lack both phase Ⅲ clinical trial validation and standardized treatment protocols.In this review,we summarize the clinical challenges and research advancements related to radiotherapy-induced xerostomia,focusing on aspects such as its pathogenesis,diagnostic and assessment tools,disease management strategies,and prediction models.Additionally,we highlight recent achievements in pathogenesis research,the application of assessment tools,the exploration of management strategies,and the development of prediction models,providing a comprehensive and forward-looking reference for the precise and personalized treatment of radiotherapy-induced xerostomia.关键词
