Current Situation and Challenges of Off-Label Drug Use Based on a Qualitative Interview

Abstract
Objective To improve the safe and rational drug use in the clinic in a hospital.Methods A qualitative research method was used to conduct"one-on-one"semi-structured in-depth interviews with 16 physicians from the Jining No.1 People's Hospital to collect physicians'cognition,experiences of off-label drug use,difficulties encountered in off-label drug use,and targeted suggestions.Colaizzi seven-step analysis method was used to classify and organize the data,and NVivo 11.0 qualitative software was used to code as the themes and sub-themes to understand the current situation and challenges of off-label drug use in the hospital.Results Four themes(physicians'understanding and practice of off-label drug use;risk awareness of off-label drug use,filing process;management and training of off-label drug use;support and assistance required by physicians)and nine sub-themes were summarized.Currently,the phenomenon of off-label drug use was widespread in clinical practice,and there were some problems such as differences in physicians'understanding of off-label drug use,incomplete drug rule libraries,and inadequate implementation of informed consent forms for off-label drug use in the outpatient department.Conclusion It is recommended to strengthen the training of physicians'relevant professional knowledge,adopt appropriate hierarchical management,regularly update medication information of off-label drug use,and cooperate with multiple departments to ensure the rights and interests of hospitals,physicians,and patients,and promote safe and rational drug use in the clinic.关键词
