Abstract

OBJECTIVE To observe the effects of the Shugan Jianpi Formula(SGJP)on active-phase lesions in a thyroid-associated ophthalmopathy(TAO)mouse model induced by the thyroid-stimulating hormone receptor(TSHR).METHODS The TAO model was established by intramuscular injection of Ad-TSHRA adenovirus into the bilateral femoral muscles of mice.The animals were randomly divided into six groups(12 mice per group):Model group(MG),prednisone group(PG),and SGJP low-,medium-,and high-dose groups(SGJP-L,SGJP-M,SGJP-H),with a control group(CG)and a negative control group(NCG).The PG group received prednisone acetate tablets(7.8 mg/kg)by gavage,while the SGJP-L,SGJP-M,and SGJP-H groups received SGJP decoction at doses of 50,100,and 200 g/kg,respectively.The CG,NCG,and MG groups were administered equal volumes of distilled water.All groups were treated continuously for 30 days.Ocular appearance was recorded photographically,and orbital T2-weighted fat-suppressed magnetic resonance imaging(MRI)was performed.Serum levels of thyroid-stimulating hormone(TSH),thyroxine(T4),and TSH receptor antibody(TRAb)were determined by enzyme-linked immunosorbent assay(ELISA).Glycosaminoglycan(GAG)expression and orbital adipose tissue weight were measured.Histopathological changes in thyroid and orbital tissues were observed by hematoxylin-eosin(HE)staining.RESULTS(1)Ocular manifestations:Approximately 62%of model mice exhibited orbital changes consistent with TAO.After treatment,periorbital edema and conjunctival congestion were alleviated in all medication groups compared with the MG group,while palpebral fissure widening,exophthalmos,and corneal ulceration showed no significant improvement.(2)Extraocular muscle morphology:The MG group showed moderate hyperintensity of extraocular muscles on MRI,suggesting hypertrophy.All treatment groups exhibited reduced proliferation of orbital adipose tissue compared with the MG group.(3)Thyroid function indices:In the MG group,TRAb and T4 levels were higher and TSH levels lower than those in the CG and NCG groups.The PG group showed lower TRAb and T4 and higher TSH levels compared with the MG group.TRAb levels were decreased in the SGJP-L and SGJP-M groups,with increased TSH in the SGJP-L group;T4 levels in the SGJP-L group were higher than in the PG group.In the SGJP-M and SGJP-H groups,TRAb and T4 were higher and TSH lower than in the PG group(all P<0.05).(4)Orbital adipose tissue and GAG expression:The MG group showed greater retrobulbar adipose tissue mass,higher adipose tissue ratio,and elevated GAG expression compared with the CG and NCG groups.These indices were significantly reduced in the PG group versus the MG group.The SGJP-L group exhibited reduced retrobulbar fat mass and GAG expression compared with the MG group,though GAG levels remained higher than those in the PG group.The SGJP-M and SGJP-H groups showed lower GAG expression than the MG group,yet higher retrobulbar adipose tissue weight and GAG expression than the PG group(all P<0.05).(5)Thyroid histopathology:The MG group showed marked thyroid enlargement,congestion,follicular cell hypertrophy and hyperplasia,and lymphocytic infiltration.These pathological changes were ameliorated in all treatment groups,with reduced congestion and edema,more uniform colloid distribution,and minimal lymphocytic infiltration.(6)Retrobulbar morphology:The number and size of retrobulbar adipocytes were reduced in all treated groups compared with the MG group.CONCLUSIONS SGJP may ameliorate active-phase lesions of TAO in mice by regulating thyroid immune-inflammatory responses,inhibiting adipocyte proliferation,and reducing glycosaminoglycan expression.
