针刀松解合阳内调控中重度膝骨关节炎患者膝周肌电信号的临床研究
Clinical Study on Regulation of Periarticular Electromyographic Signals in Patients with Moderate to Severe KOA by Acupotomy Releasing Heyangnei

Abstract
Objective:To explore the patterns of periarticular electromyographic signals in patients with moderate to severe knee osteoarthritis(KOA)treated with acupotomy releasing Heyangnei.Methods:A total of 60 patients with medial compartment KOA of Kellgren-Lawrence grade Ⅲ-Ⅳ were randomly divided into the acupotomy group and the sham acupotomy group.Both groups received identical health education and basic treatment.The acupotomy group underwent acupotomy intervention at Heyangnei,while the sham group received non-penetrative sham acupotomy at the same point.Treatment was administered once per week for a total of three weeks(one treatment course).Overall clinical efficacy was evaluated using the WOMAC score.Electromyographic(EMG)signals of periarticular muscles were measured before the treatment,after the treatment,and during the follow-up to assess changes.Results:The overall clinical efficacy in the acupotomy group was 73.10%(19/26),which was significantly higher than 16.00%(4/25)in the sham acupotomy group(P<0.01).After the treatment and within the follow-up,the integrated electromyography(IEMG)and median frequency(MF)values of major periarticular knee muscles during standing and walking were significantly higher in the acupotomy group than those in the sham acupotomy group(P<0.05,P<0.01).During maximal resistance knee extension,the IEMG and MF values of the vastus lateralis,rectus femoris,tibialis anterior,medial gastrocnemius,and lateral gastrocnemius were significantly higher in the acupotomy group than those in the sham acupotomy group(P<0.05,P<0.01).During maximal resistance knee flexion,the IEMG and MF values of the tibialis anterior,biceps femoris,semitendinosus,medial gastrocnemius,and lateral gastrocnemius were also significantly higher in the acupotomy group than those in the sham acupotomy group(P<0.05,P<0.01).Conclusion:Acupotomy releasing Heyangnei can stimulate Yang Qi around the knee,nourish the tendons and muscles,increase the IEMG and MF values of periarticular muscles,improve muscle strength and fatigue resistance,and help restore the balance between tendons and bones of the knee joint,thereby achieving the therapeutic effect of relaxing tendons and easing joint movement.关键词
