基于心率变异性监测电针对胃癌术后胃肠功能康复的临床研究
Electro-Acupuncture for Gastrointestinal Function Recovery after Gastric Cancer Surgery:A Clinical Study Based on Heart Rate Variability Monitoring

Abstract
Objective:To investigate the efficacy and safety of electro-acupuncture(EA)in promoting postoperative gastrointestinal function recovery in gastric cancer patients,and to explore its possible mechanisms based on heart rate variability(HRV)indicators.Methods:This prospective randomized controlled trial included 80 patients who underwent gastric cancer surgery.The patients were randomly assigned to the EA group(n=40)and the sham EA group(n=40).Both groups received perioperative management based on the Enhanced Recovery After Surgery(ERAS)protocol;on which basis,the EA group received EA stimulation at bilateral Zusanli(ST36),Sanyinjiao(SP6),and Hegu(LI4)for 30 minutes at 2 hours postoperatively and on the morning of postoperative days 1 and 2,while the sham EA group received sham electro-needling.Real-time monitoring was performed using an AI-based HRV device and a continuous bowel sound recording system.The following outcomes were observed and compared between the two groups:gastrointestinal function recovery indicators(time to first flatus,time to first oral intake,time to bowel sound recovery,and postoperative hospital stay),HRV parameters(SDNN,SDNN Index,pNN50,HF,LF),inflammatory markers(WBC,CRP,IL-1 β,IL-2,IL-6,TNF-α,IFN-γ),as well as postoperative complications and adverse events.Results:The EA group showed significantly faster recovery of gastrointestinal function compared with the sham EA group,including shorter time to first flatus,first oral intake,bowel function recovery,and postoperative hospital stay(P＜0.05).On postoperative day 3,inflammatory markers(CRP,WBC,IL-1 β,IL-6)were significantly lower in the EA group than in the sham EA group(P＜0.05).At the same time point,HRV parameters(SDNN Index,pNN50,HF)were significantly higher in the EA group than in the sham EA group(P＜0.05).No significant differences were observed between the two groups in baseline characteristics,preoperative inflammatory markers,preoperative HRV parameters,postoperative day 1 inflammatory markers,or postoperative complications and adverse events(P＞0.05).Conclusion:EA can safely and effectively promote gastrointestinal function recovery in patients after gastric cancer surgery.Its potential mechanism may be related to vagus nerve activation,improvement of vagal tone,and reduction of inflammatory levels.关键词
