Diagnostic value of placental morphological parameters measured by three-dimensional ultrasound for early-onset fetal growth restriction

Abstract
Objective To evaluate the diagnostic value of placental morphological parameters measured by three-dimensional ultrasound for early-onset fetal growth restriction(EFGR)in high-risk birth-giving women.Methods This study used a prospective,cross-sectional cohort observation,randomly selected 105 high-risk pregnant women(advanced age,obese,hypertension during pregnancy,and gestational diabetes)with normal delivery in Jinhua Maternal and Child Health Care Hospital from June 2023 to June 2024,all of whom were given single live fetus.The gestational age calculated by ultrasound was 13-27 weeks.According to the weight of postpartum newborns,there were 34 cases in the EFGR group and 71 cases in the normal group.Conventional ultrasound was used to estimate fetal weight(EFW),while three-dimensional ultrasound was used to measure placental morphological parameters including length,width,thickness,surface area,and volume.Placental blood flow parameters included vascular index(VI),flow index(FI),and vascular blood flow index(VFI).The ratio of above parameters to gestational age was standardized.ROC curve was drawn to evaluate the performance of placental morphological parameters in predicting EFGR,and AUC was calculated.Results Compared with the normal group,there were no significant differences of EFW,standardized placental width and thickness,VI,FI,and VFI values in the EFGR group(all P>0.05),but standardized placental length,surface area,and volume were significantly lower(all P<0.05).ROC showed that AUC of standardized placental length for predicting FGR was 0.944(95%CI:0.903-0.986,P<0.001),significantly higher than standardized placental surface area and volume(both P<0.001).The optimal critical value of standardized placental length was 2.67,indicating a risk of EFGR if<2.67.Conclusion The use of placental morphological parameters measured by three-dimensional ultrasound during pregnancy,especially standardized placental length,has better diagnostic performance than conventional ultrasound EFW in high-risk pregnant women with EFGR,which can be used as a new non-invasive indicator for prenatal diagnosis.关键词
