长轴穿刺对立体定向脑出血手术血肿清除率的影响
Effeef of long-axis puncture on hematoma clearance in stereotactic surgery for intracerebral hemorrhage

Abstract
Objective To explore the effect of long-axis puncture on hematoma clearance in stereotactic surgery for intracerebral hemorrhage.Methods Clinical and imaging data of 144 patients with spontaneous intracerebral hemorrhage who underwent stereotactic hematoma evacuation at Ningbo Medical Center Lihuili Hospital between January 2019 and December 2023 were retrospective analyzed.During the stereotactic surgery the puncture trajectory was followed the long axis of the hematoma in 51 cases(long-axis puncture group)and did not follow the long axis in 93 cases(non-long-axis puncture group).Hematoma volume and surface area were calculated using 3D Slicer software.Postoperative hematoma clearance rate and residual hematoma volume were compared between the two groups.Results The hematoma clearance rate in the long-axis puncture group(66.7％±19.6％)was significantly higher than that in the non-long-axis group(53.6％±26.4％),(P=0.016).The postoperative residual hematoma volume in the long-axis group[10.7(6.2,18.4)mL]was significantly lower than that in the non-long-axis group[20.3(6.4,26.7)mL](P=0.041).Conclusion Puncturing along the long axis of the hematoma can significantly increase hematoma clearance rate and reduce residual hematoma volume for intracerebral hemorrhage patients undergoing stereotactic hematoma evacuation surgery.关键词
