超声引导下针刀联合神经阻滞术治疗腰椎间盘突出症的比较研究

Abstract
Objective:To compare and analyze the clinical effect of ultrasound-guided needle-knife therapy combined with nerve block in the treatment of lumbar disc herniation(LDH).Method:50 patients with LDH admitted to the First Department of Orthopedics,Heilongjiang Academy of Chinese Medicine Sciences from January 2024 to January 2025 were selected as the research subjects.They were evenly divided into two groups by random number table method,with 25 cases in each group.The control group were treated with ultrasound-guided needle-knife release surgery,while the experimental group were treated with ultrasound-guided needle-knife release combined with nerve block.Both groups of patients received a single treatment uniformly.The Visual Analogue Scale(VSA)score,Japanese Orthopaedic Association Assessment of Treatment(JOA)score,and Lumbar Dysfunction Index(ODI)score were evaluated before treatment,3 days after treatment,and 2 weeks after treatment,respectively,to assess the changes in the condition and the therapeutic effect.Result:Compared with before treatment,the VAS and ODI scores of both groups at 3 days and 2 weeks after treatment were lower than those before treatment,while the JOA score was higher.Moreover,the VAS and ODI scores of the experimental group were lower than those of the control group,and the JOA score was higher than that of the control group.The differences were statistically significant(P<0.05).The clinical efficacy of the experimental group was better than that of the control group,and the difference was statistically significant(P<0.05).Conclusion:Needle-knife release for the treatment of LDH can produce better analgesic and functional improvement effects,and the combination of needle-knife release and nerve block can further enhance the therapeutic effect.Moreover,no serious complications such as vascular and nerve injuries occurred,confirming its clinical safety.关键词
