ALBI评分联合降钙素原预测肝硬化合并急性肾损伤的临床价值
Clinical value of albumin-bilirubin score combined with PCT in the prediction of cirrhosis combined with acute kidney injury

Abstract
Objective:The incremental value of combining the albumin-bilirubin(ALBI)score with serum procalcitonin(PCT)for predicting acute kidney injury(AKI)in patients with cirrhosis will be evaluated in our study.Methods:The clinical data of 336 patients with cirrhosis who were hospitalized between June 2020 and June 2024 were analyzed in this retrospective study.Based on the occurrence of acute kidney injury(AKI)within seven days of hospitalization,patients were divided into the cirrhosis-AKI group(87 cases)and the cirrhosis-non-AKI group(249 cases).Multivariate logistic regression was used to identify the factors influencing the development of AKI in cirrhotic patients.Additionally,the predictive performance of the ALBI score combined with PCT for AKI was assessed using receiver operating characteristic(ROC)curves,with the area under the curve(AUC)representing the results.Results:The ALBI score and PCT levels were significantly higher in the cirrhosis-AKI group than in the non-AKI group(P<0.05).Patients in the AKI group were more likely to be older than 65 years and have hepatic encephalopathy or spontaneous peritonitis,as well as elevated serum uric acid,prothrombin time,and IL-6 levels,which may represent potential confounding factors for AKI development.Multivariate logistic regression analysis revealed that the ALBI score(OR=1.637,95%CI:1.262-2.124)and PCT(OR=1.358,95%CI:1.065-1.732)were independent risk factors for AKI in patients with cirrhosis.Receiver operating characteristic(ROC)curve analysis showed that the combination of ALBI score and PCT achieved an AUC of 0.885(95%CI:0.847-0.923),with a sensitivity of 83.91%and specificity of 77.11%,indicating superior predictive performance compared with either indicator alone.Conclusion:The ALBI score and PCT were identified as independent risk factors for AKI in patients with cirrhosis.The combined assessment of ALBI score and PCT enables early prediction of AKI risk,facilitates the identification of highrisk patients,and may assist clinicians in implementing timely interventions.关键词
