托毒排脓方联合穿刺抽脓术治疗成脓期肉芽肿性小叶性乳腺炎临床研究
Clinical study on Tuodu Painong Formula combined with puncture and aspiration in treating granulomatous lobular mastitis in the suppurative stage

Abstract
Objective To explore the clinical efficacy of Tuodu Painong Formula combined with puncture and aspiration for treating granulomatous lobular mastitis(GLM)in the suppurative stage.Methods A total of 102 GLM patients in the suppurative stage admitted to Department of Breast Surgery,Jinjiang City Hospital/Fujian Hospital of Shanghai Sixth People's Hospital from January 2021 to November 2024 were selected and divided into the control group and the treatment group according to random number tabel method.The control group received puncture and aspiration alone,while the treatment group received puncture and aspiration combined with oral administration of Tuodu Painong Formula.The treatment course for both groups was 8 weeks.Traditional Chinese medicine(TCM)syndrome scores(refractory abscess rupture,indurated mass,fatigue,pale complexion),cross-sectional area of breast mass,volume of breast abscess cavity,pain severity[(Visual Analogue Scale,VAS)score],blood routine parameters[white blood cell count(WBC),neutrophil percentage(NEUT%),erythrocyte sedimentation rate(ESR)],laboratory indicators[prolactin(PRL),interleukin(IL)-6,IL-10,C-reactive protein(CRP)],incidence of adverse reactions,and the total effective rate in both groups were compared.Results After treatment,TCM syndrome scores(refractory abscess rupture,indurated mass,fatigue,pale complexion),cross-sectional area of breast mass,volume of breast abscess cavity,VAS score,levels of WBC,NEUT%and ESR,and levels of PRL,IL-6,IL-10 and CRP in both groups decreased,and the treatment group was lower than the control group(P<0.05).The total effective rate(clinical recovery+effective)in the treatment group was significantly higher than that in the control group(P<0.05).There was no statistically significant difference in the total incidence of adverse reactions between two groups(P>0.05).Conclusion The combination of Tuodu Painong Formula and puncture and aspiration demonstrates outstanding efficacy in treating suppurative-stage GLM.This therapy can rapidly alleviate patients'subjective symptoms,reduce the volume of abscess cavity and the area of mass,relieve pain,inhibit inflammatory responses,and enhance clinical efficacy.关键词
