通督调神针法联合西药治疗抑郁症伴失眠的疗效观察及对血清SOD和BDNF水平的影响
Observation on the efficacy of Tong Du Tiao Shen acupuncture combined with western medicine in the treatment of depression with insomnia and its effect on serum SOD and BDNF levels

Abstract
Objective To observe the clinical efficacy of Tong Du Tiao Shen acupuncture combined with agomelatine in the treatment of depression with insomnia,and to evaluate its effects on serum superoxide dismutase(SOD)and brain-derived neurotrophic factor(BDNF)levels.Method A total of 85 patients with depression and insomnia were randomly assigned to the acupuncture group(42 cases)and the control group(43 cases).The control group received sham acupuncture combined with agomelatine,while the acupuncture group received Tongdu Tiaoshen acupuncture combined with agomelatine.Clinical efficacy were compared between the two groups,and Pittsburgh Sleep Quality Index(PSQI),17-item Hamilton Depression Scale(HAMD-17)scores,serum biomarkers[SOD,BDNF,melatonin,glial-cell-line-derived neurotrophic factor(GDNF),noradrenaline(NE),5-hydroxytryptamine(5-HT),dopamine(DA),acetylcholine(ACh)],polysomnographic parameters[slow-wave sleep(SWS)duration,wake time,rapid eye movement(REM)sleep duration,sleep latency(SL),total sleep time(TST),number of awakenings,sleep efficiency(SE)],and adverse events were compared before and after treatment between the two groups.Result The total effective rate was 92.9%in the acupuncture group and 74.4%in the control group,with the acupuncture group showing a significantly higher rate(P＜0.05).After treatment,PSQI and HAMD-17 scores decreased in both groups,with lower scores in the acupuncture group than in the control group(P＜0.05).Serum biomarker levels increased in both groups after treatment,with significantly higher levels in the acupuncture group(P＜0.05).Compared with baseline,REM sleep time,SWS time,SE,and TST increased,while wake time,number of awakenings,and SL decreased in both groups;improvements were more pronounced in the acupuncture group(P＜0.05).The incidence of adverse reactions showed no significant difference between groups(P＞0.05).Conclusion Tong Du Tiao Shen acupuncture combined with agomelatine demonstrates superior efficacy in the treatment of depression with insomnia,improving sleep quality,regulating oxidative stress,and enhancing neurological function.关键词
