Abstract
Objective To establish a Nomogram model for predicting asthma in children with acute lower respiratory tract infection.Methods A total of 360 patients with acute lower respiratory tract infection who were treated in our hospital from January 2022 to June 2024 were included.According to whether they were complicated with asthma,they were divided into asthma group and non-asthma group.Clinical data of the two groups were collected and compared between groups.Multivariate Logistic regression analysis was performed on statistically significant univariate variables.Logistic regression was used to analyze the related factors affecting asthma in children with acute lower respiratory tract infection.Nomogram prediction model was constructed by using R language.ROC,calibration curve and decision curve were used to evaluate and analyze the model.Results Age,history of wheezing,allergy,family history of asthma,smoking environment,long time of fever remission and cough disappearance,and high IL-6 level were the related factors of asthma in children with acute lower respiratory tract infection.The AUC values of age,fever remission time,cough disappearance time and IL-6 were 0.740,0.818,0.877,and 0.936,respectively,suggesting that age,fever remission time,cough disappearance time and IL-6 had a certain ability to distinguish whether asthma was complicated.Multivariate analysis showed that age,wheezing history,allergy,family history of asthma,smoking environment,time of fever remission,cough remission time and IL-6 were risk factors for asthma in children with acute lower respiratory tract infection.The constructed prediction model performed well,C index was 0.997(0.992-1.002),indicating that there was no significant difference between the predicted value and the observed value.The calibration curve showed that there was a good consistency between the observation and prediction levels of the data.Conclusion Age,history of wheezing,allergy,family history of asthma,smoking environment,time of fever remission,time of cough remission and IL-6 are risk factors for asthma in children with acute lower respiratory tract infection.The constructed prediction model performs well and has certain clinical significance.关键词
