二维超声联合超声弹性成像在乳腺肿块良恶性鉴别诊断中的应用价值
Application Value of Two-Dimensional Ultrasound Combined with Ultrasonic Elastography in the Differential Diagnosis of Benign and Malignant Breast Masses

Abstract
Objective To explore the value of two-dimensional ultrasound(2D-US)combined with ultrasound elastography(UE)in differential diagnosis of benign and malignant breast masses.Methods The clinical data of 112 patients with breast masses admitted to the First Affiliated Hospital of Nanyang Medical College from April 2022 to December 2024 were retrospectively analyzed,and all patients received 2D-US and UE examinations.Taking the pathological examination results as the"gold standard"for diagnosing the nature of breast masses,the efficacy of single and combined examinations of 2D-US and UE in diagnosing benign and malignant breast masses was analyzed,and the imaging features,elastic strain rates(SR),and UE scores of breast masses with different natures were compared.Results Pathological examination results showed that 139 lesions were detected in 112 patients with breast masses,including 93 benign lesions and 46 malignant lesions.The sensitivity,specificity,accuracy,positive predictive value and negative predictive value of combined examination of 2D-US and UE in the diagnosis of benign and malignant breast masses were 95.65%,96.77%,96.40%,93.62%and 97.83%,respectively,higher than those of single examination(P<0.05).Kappa test showed that the diagnostic result of 2D-US combined with UE examination had good consistency with pathological examination result(Kappa value=0.919,P<0.001),while the consistency of the individual diagnostic results of 2D-US or UE with the pathological examination result was acceptable(Kappa value=0.573,0.643;both P<0.001).The proportions of irregular shape,unclear boundary,microcalcification,capsule,and absent or hypoechoic internal echoes of malignant masses were higher than those of benign masses(P<0.05).The morphological SR of malignant masses was lower than that of benign masses,while the UE score was higher than that of benign masses(P<0.05).Conclusions Both 2D-US and UE are effective methods for the clinical differential diagnosis of benign and malignant breast masses.The combined examination of the two has higher efficacy in the differential diagnosis of benign and malignant breast masses.关键词
