多模态超声技术在肾透明细胞癌与肾血管平滑肌脂肪瘤鉴别诊断中的效能分析
Analysis on the Efficacy of Multimodal Ultrasound Technique in the Differen-tial Diagnosis of Clear Cell Renal Cell Carcinoma and Renal Angiomyolipoma

Abstract
Objective To explore the efficacy of multimodal ultrasound technique in the differential diagnosis of clear cell renal cell carcinoma(ccRCC)and renal angiomyolipoma(RAML).Methods 110 patients with renal space occupying lesions who visited the urology department of our hospital from January 2021 to May 2025 were selected.All patients were diagnosed through surgical pathology or biopsy,including 62 patients with ccRCC and 48 patients with RAML.The conventional ultrasound characteristics,hemodynamics,and contrast-enhanced ultrasound features of two groups were compared.With pathological diagnosis or biopsy diagnosis results as the gold standard,the diagnostic efficacy of conventional ultrasound and multimodal ultrasound technology in distinguishing the two types of lesions was calculated.Results The lesion diameter in the ccRCC group was larger than that in the RAML group,and the proportions of irregular shape,blurred boundaries,and low/mixed echo were significantly higher than those in the RAML group(P<0.05).The Adler blood flow grading,high-speed and high resistance blood flow,arterial phase enhancement start time,enhancement peak time,enhancement degree,enhancement uniformity,capsule enhancement,and enhancement resolution mode were statistically different between the ccRCC group and the RAML group(P<0.05).Using pathological diagnosis or biopsy diagnosis results as the gold standard,multimodal ultrasound technique had significantly higher sensitivity,specificity,positive predictive value,negative predictive value,and accuracy in the differential diagnosis of ccRCC and RAML than conventional ultrasound(P<0.05).Conclusions Multimodal ultrasound technique has significant differential diagnostic value for clear cell renal cell carcinoma and renal angiomyolipoma by integrating information such as lesion structure and blood flow perfusion,which can be used as an effective method for accurately distinguishing the two types of renal space occupying lesions in clinical practice.关键词
