LISA技术联合CPAP治疗早产儿呼吸窘迫综合征的效果及对脑氧代谢的影响研究
Study on the Effect of LISA Technique Combined with Continuous Positive Airway Pressure in the Treatment of Premature Infants with Respiratory Dis-tress Syndrome and the Impact on Cerebral Oxygen Metabolism

Abstract
Objective To study the effect of less invasive surfactant administration(LISA)technique combined with continuous positive airway pressure(CPAP)in the treatment of premature infants with respiratory distress syndrome(RDS)and the impact on cerebral oxygen metabolism.Methods 40 RDS premature infants admitted to neonatal intensive care unit(NICU)of our hospital from January 2024 to October 2024 were selected and randomly divided into control group(CPAP)and observation group(LISA technique combined with CPAP),with 20 cases in each group.The treatment status,hospitalization time,cerebral oxygen indicators[cerebral oxygen saturation(ScO2)],blood gas analysis indicators[partial pressure of carbon dioxide(PaCO2),oxygenation index(PaO2/FiO2),pH value],and adverse reactions were compared between the two groups.Results The intubation time,oxygen use time,and hospitalization time of the observation group were significantly shorter than those of the control group,and the re-use rate of pulmonary surfactant(PS)and the mechanical ventilation rate within 72 hours were significantly lower than those of the control group(P<0.05).5 minutes after administration,the ScO2 of the observation group was significantly lower than that of the control group(P<0.05).After treatment,the PaCO2 of the observation group was significantly lower than that of the control group,and the PaO2/FiO2 and pH value were significantly higher than those of the control group(P<0.05).The incidence of adverse reactions of the observation group was 0.00%,significantly lower than 25.00%of the control group(P<0.05).Conclusions LISA technique combined with CPAP in the treatment of premature infants with RDS can shorten the intubation time,oxygen use time and hospitalization time,reduce the re-use rate of PS and mechanical ventilation rate,improve the cerebral oxygen metabolism and pulmonary oxygenation,and reduce the occurrence of adverse reactions,which is worthy of clinical promotion and application.关键词
