Survey on the Operation Status and Drug Allocation Models of Pharmacy Intravenous Admixture Services in Medical Institutions of Hebei Province

Abstract
Objective To improve the operation efficiency and management quality of pharmacy intravenous admixture services(PIVAS).Methods Based on the Guidelines for the Construction and Management of Pharmacy Intravenous Admixture Services(Trial),a questionnaire was designed combined with the operation status of PIVAS in medical institutions of Hebei Province,the content included basic information of hospitals and basic status,personnel situation,work content,anti-tumor drug allocation process,quality control of PIVAS,etc.An online survey was conducted by ″Questionnaire Star″ wechat mini program in July 2023,and Excel software was used for data statistical analysis.Results A total of 38 valid questionnaires were collected from medical institutions(including 21 tertiary Grade A,10 tertiary Grade B,and 7 secondary Grade A hospitals),all of which had established PIVAS.Among which,the number of PIVAS in Tangshan City and Xingtai City was the highest(both had 7 institutions,18.42%).The average activation time of PIVAS was 75.21 months,with an average construction area of 641.79 m²,average clean zone area of 136.80 m²,average number of service beds of 974.95,daily average allocation amount of 2 382.25 bags,and per capita allocation amount of 92.39 bags.The construction area was highest in the range of 300-<500 m²(12 institutions,31.58%).In terms of equipment configuration,all 38 institutions were equipped with horizontal laminar flow cabinets(average 7.71 units),36 institutions were equipped with Class Ⅱ Type A2 biosafety cabinets(average 5.89 units),and 5 institutions were equipped with Class Ⅱ Type B2 biosafety cabinets(average 2.80 units).The automated equipment was mainly automatic dispensing machines(14 institutions,36.84%)and automatic labeling machines(11 institutions,28.95%).In terms of operation mode,the collaborative mode of ″pharmacist+nurse+general worker″ was mainly adopted(32 institutions,84.21%).The daily average allocation volume was mostly 1 001-2 000 bags(17 institutions,44.74%).Problems existed in the allocation of anti-tumor drugs,such as inconsistent standards for drug variety classification,non-strict compliance with instructions in the use of pre-solvents,inadequate implementation of residual drug sharing policies,and insufficient protective measures.Conclusion The construction of PIVAS in medical institutions of Hebei Province has achieved initial results,but there are still many problems in operation and management.It is necessary to further strengthen the quality of operation management,improve the efficiency of drug preparation and the level of information construction,and enhance the awareness of occupational protection among medical staff.关键词
