静脉用药调配中心不合理医嘱干预效果分析
Analysis of the Effect of Intervention for Irrational Medical Orders in the PIVAS

Abstract
Objective To improve the rational rate of medical orders for intravenous medication.Methods All medical orders from the pharmacy intravenous admixture service in the hospital's prescription review system from January to December 2023 were taken as the observation group(331 317 orders,receiving routine interventions and pharmacist interventions).Orders from January to December 2022 were taken as the control group(353 779 orders,receiving routine interventions).Reviewing irrational medical orders,conducting medication knowledge training,developing standardized order templates,defining prescription review rules,and implementing secondary review were included in pharmacist interventions.The incidence,type distribution,and departmental distribution of irrational medical orders were compared between the two groups.Results The rate of irrational medical orders in the observation group was 1.43%(4 730/331 317),which was significantly lower than 1.82%(6 455/353 779)in the control group(χ²=167.865,P<0.05).Among which,irrational medical orders such as lack of sterile water for injection,lack of skin test,inappropriate solvent dosage,incorrect administration frequency,and lack of order remarks were significantly improved(P<0.05).The Departments with more irrational medical orders were mainly Pediatrics,Respiratory Medicine,and Oncology.Conclusion Pharmacist interventions can significantly reduce the rate of irrational medical orders and effectively promote the standardized use of intravenous infusions.关键词
