双相情感障碍患儿体内拉莫三嗪血浆谷浓度与ABCB1 C3435T基因多态性的相关性研究
Study on the Correlation Between Lamotrigine Plasma Trough Concentration and ABCB1 C3435T Gene Polymorphism in Children with Bipolar Disorder

Abstract
Objective To investigate the correlation between lamotrigine plasma trough concentration and ABCB1 C3435T gene polymorphism in children with bipolar disorder.Methods A total of 102 children with bipolar disorder who were treated with lamotrigine for more than one month in the People's Hospital of Xinjiang Uygur Autonomous Region from January 2020 to December 2023 were selected retrospectively.Ultra performance liquid chromatography method was used to determine the lamotrigine plasma trough concentration in children.Sanger sequencing method was used to detect the ABCB1 genotype.The lamotrigine plasma trough concentration was standardized by the concentration/dose value of body mass normalization.The correlation between the ABCB1 gene polymorphism and the lamotrigine plasma trough concentration in children was analyzed.Results A total of 102 children were included,mainly female children(82 cases,80.39%);the average dosage was(3.96±2.22)mg/(kg·d);the average plasma trough concentration was(4.16±2.44)µg/mL.Gender was the potential influencing factor for the lamotrigine plasma trough concentration in children(P<0.05).In addition,the lamotrigine plasma trough concentration in children was significantly positively correlated with the dosage(r=0.527,P=0.000).The distribution of genotype of ABCB1 G2677T/A and C3435T conformed to Hardy-Weinberg equilibrium,which indicated that the subjects were representative of the population.There was no significant difference in the plasma trough concentration among children with different genotypes of ABCB1 G2677T/A(P>0.05).There were significant differences in the plasma trough concentrations among children with different genotypes of ABCB1 C3435T(P<0.05),and the plasma trough concentrations of children with CT and TT genotype were significantly lower than those of children with CC genotype(P<0.05).Conclusion The ABCB1 C3435T gene polymorphism in children with bipolar disorder can significantly affect the plasma trough concentration of lamotrigine.It is suggested that the ABCB1 C3435T gene polymorphism in children should be detected before lamotrigine treatment,and the medication regimen should be individually adjusted in time according to the test results.关键词
