Abstract
Cancer-related fatigue is a subjective sense of fatigue caused by tumors itself or related treatments.It often appears as a concomitant symptom in patients with advanced tumors in clinical practice,has a high incidence rate,cannot be relieved by rest,and effective clinical intervention methods need to be developed.Cancer-related fatigue falls within the category of"consumptive disease"in traditional Chinese medicine,presenting symptoms such as poor appetite,weakness,depression,dry heat,or insomnia.Patients with advanced tumors have weakened vital Qi(气),and the damage caused by drug toxins further depletes their vital Qi.The spleen and stomach are injured,which is the root cause of the pathogenesis of cancer-related fatigue.Internal injury of the spleen and stomach leads to the inability of the essence and micro-substances of food and water to be distributed,causing them to flow down to the kidneys.The ascending Qi in the lower energizer is blocked,and Yin(阴)fire accumulates internally,which is the transformation mechanism of disease progressi.The body is in a state of deficiency and damage,with sluggish Qi and blood circulation due to blocked Qi circulation,and the influences of additional pathological factors such as phlegm,dampness,blood stasis,and toxins,causing the pathological characteristics of deficiency and stagnation throughout the whole process of occurrence and progression of the disease.In this study,based on LI Gao's theory of the relationship between Qi and fire of"Fire and vital Qi cannot coexist;when one prevails,the other declines",it is believed that the essence of cancer-related fatigue is"Fire prevails and vital Qi declines".In clinical practice,to rescue the deficient vital Qi,eliminate Yin fire,and relieve fatigue symptoms,it is necessary to first ensure the normal functions of the spleen and stomach and the abundance of vital Qi.In this way,Yin fire will naturally subside.Therefore,the treatment principles of"tonifying the spleen and stomach to enrich vital Qi","elevating clear Yang(阳)and purging Yin fire",and"smoothing Qi circulation to unblock stagnation"are adopted clinically.The treatment is from the perspective of the relationship between Qi and fire,with the purposes of ameliorating the state of deficiency and damage of internal organs in patients,alleviating the symptoms of cancer-related fatigue,and enhancing their quality of life,thereby expanding the treatment ideas for cancer-related fatigue in traditional Chinese medicine.关键词
