基于《素问·痿论篇》探析针刺治疗肌萎缩侧索硬化的思路
Exploring the idea of acupuncture in the treatment of amyotrophic lateral sclerosis based on chapter Weilun of Suwen

Abstract
Amyotrophic lateral sclerosis is a progressive neurological disease involving upper and lower motor neurons,with progressive aggravation of skeletal muscle weakness,muscle atrophy,fasciculation,medullary palsy,and pyramidal track sign as the main clinical manifestations.At present,the disease cannot be cured yet,and only medicines can be used to delay its progression,which is a dif-ficult problem in the world.Amyotrophic lateral sclerosis belongs to the category of"Wei(痿)syndrome"in traditional Chinese medicine.Chapter Weilun of Suwen(《素问·痿论篇》)proposed the treatment idea of"treating Wei syndrome by taking Yangming(阳明)merid-ians alone"and"all five internal organs can cause Wei syndrome".Guided by the theory of Wei syndrome in chapter Weilun of Suwen,this paper discusses the ideas and methods of acupuncture differentiation and treatment of amyotrophic lateral sclerosis.Based on the theory,the core mechanism of amyotrophic lateral sclerosis is considered as Yangming deficiency,Qi(气)and blood deficiency,loss of muscle nourishment,and even atrophy.Yangming refers to a series of meridians with rich Qi and blood.In terms of treatment,acupuncture point selection should focus on the acupoints of the Yangming meridian.Needling Yangming acupoints can nourish Qi and blood to moisten the tendons,stimulate the Qi of the Yangming meridian to nourish the tendons and vessels,and delay skeletal muscle atrophy.For amyotrophic lateral sclerosis with dyspnea,muscle bundle fibrillation,cognitive impairment and other complications,we should flexibly select acupoints according to the different stages of the disease and the corresponding symptoms.We should mainly select Yangming meridians while tak-ing care of the essence and Qi of the five internal organs.For cases of amyotrophic lateral sclerosis accompanied by bulbar paralysis,the role of the Ren(任)and Du(督)meridians should be emphasized.By regulating the Ren and Du meridians,essence can be nourished and marrow can be replenished.The traditional acupuncture combined with nerve trunk stimulation technology can jointly promote Qi and blood harmony,enriching the meridians.Such treatment method can nourish the muscles,bones,and internal organs externally,and delay the progression of amyotrophic lateral sclerosis internally,providing new ideas for the clinical treatment of amyotrophic lateral sclerosis.关键词
