Abstract
Post infectious cough is a common cause of chronic cough in children,accounting for 21.73%of cases.After anti-infective treatment,although the acute symptoms are relieved,the cough is often recurrent and intractable.Professor SHENG Lixian,a famous traditional Chinese medicine practitioner in Zhejiang Province,through over 50 years of clinical practice,puts forward the"lung Qi(气)can not be dredged,spleen deficiency leads to phlegm"as the core pathogenesis,and has created the"dredging blockage in lungs to regulate the Qi method"treatment system,which has remarkable clinical efficacy.According to Prof.SHENG,children are the people of immature Yin(阴)and Yang(阳)and are susceptible to external evils,and their lungs and spleens are often deficient.After the infection,the remaining evil lingers in the lungs,the lung Qi fails to ascend and descend;the spleen is deficient,so the water and dampness stagnate and transform to phlegm;and the phlegm and Qi intermingle to cause the cough to be prolonged.The treatment should be to dredge the lung Qi as the main method,using the San'ao decoction(三拗汤)as the basic formula,accompanied by Jiegeng(Radix Platycodonis)to disperse the lung Qi,and Zhiqiao(Fructus Aurantii)to descend the Qi and unblock the middle Jiao(焦),to form the Qi regulation mode of"one ascending and one descending",and the two synergistically enhance the efficacy of dredging blockage in lung and resolving the phlegm.In terms of treatment,insect drugs such as Chantui(Periostracum Cicadae)and Jiangcan(Bombyx Batryticatus)can be used in the acute phase to play anti-allergic and anti-inflammatory effects,so as to relieve airway spasm.Patients with cough complicated with rhinitis can add Xinyi(Flos Magnoliae),Baizhi(Radix Angelicae Dahuricae)and other drugs to ameliorate postnasal drip.In the remission period,Yupingfeng San(玉屏风散)can be used to replenish Qi and solidify the exterior,regulate the immune balance,and reduce the immune response.Thus,a complete closed-loop treatment system can be formed through the"dredging blockage in lung-regulating Qi-reinforcing the vital energy".Clinical observation shows that this therapy can shorten the cough relief time and reduce the recurrence rate.Professor SHENG emphasizes that"treating cough requires treating Qi first",and attention should be paid to the application of the theory of Qi ascending and descending in the field of pediatrics.This academic thought embodies the holistic view of"treating lung and spleen together"and the concept of"treating disease before illness",which provides a new idea for the treatment of post infectious cough in children in TCM.关键词
