Screening of Core Scales for Prurigo Nodularis and Determination of Weights Based on the Delphi Method and Analytic Hierarchy Process

Abstract
Objective In order to screen the core scale for evaluating the severity of prurigo nodularis(PN)and determine its weight by using the Delphi method,expert consensus conference and analytic hierarchy process,providing a reference for the evaluation of PN.Methods Retrieved China national knowledge infrastructure(CNKI),Wanfang data knowledge service platform(Wanfang Data),VIP database(VIP),Web of Science,PubMed,and EMBASE for PN-related literature included from the establishment of these databases to November 1,2023,and constructed a item pool via literature evaluation.The Delphi method was used to screen item indicators,and the analytic hierarchy process(AHP)was employed to calculate the item weights.Finally,an expert consensus meeting was conducted to reach an agreement on the screened indicators.Results A total of 113 articles were included,and the constructed item pool contained 16 items.Three rounds of Delphi expert consultation recovered 76 valid questionnaires,and the consensus conference recovered 27 questionnaires.The expert activity coefficient was 100%,the expert authority degree was all greater than 0.80,and the Kendall coordination coefficients were 0.087,0.195 and 0.141 respectively.Through analysis of the concentration of expert opinions,14 items related to the evaluation of PN were screened out.The first level indicator weights determined by the AHP,from high to low,were pruritus,skin damage and quality of life.In terms of itch intensity,the weights were ranked from highest to lowest as follows:Peak pruritus numerical rating scale(PP-NRS),worst itch numerical rating scale(WI-NRS),visual analogue scale(VAS),and itch numerical rating scale(NRSI).For skin lesion severity,the order was:prurigo nodularis activity score(PAS),body surface area(BSA),investigator's global assessment(IGA),and prurigo nodularis impact profile(PIP)score.Regarding quality of life,the ranking was:dermatology life quality index(DQLI),itchy quality of life(Itchy QoL)scale,pittsburgh sleep quality index(PSQI),sleep disturbance numerical rating scale(SD-NRS),and sleep condition self-report scale(SRSS).The expert consensus conference determined that the recommended clinical PN evaluation scales were PP-NRS,PAS,DQLI and traditional Chinese medicine(TCM)syndrome score.Conclusion By analyzing and evaluating the scales relate to the evaluation of PN through the Delphi method,expert consensus meetings and the AHP,a core scale set for evaluating the severity of PN is established,providing a reference for the clinical assessment of PN.关键词
