A study on the mechanism of the"accelerating pus secretion to promote tissue regeneration"method in the treatment of non-puerperal mastitis wounds based on macrophage polarization

Abstract
In recent years,the incidence of non-puerperal mastitis(NPM)has exhibited a marked upward trend.The etiology remains poorly understood,and the condition can affect women of all adult age groups.The timing of breast abscess rupture is unpredictable;following spontaneous or surgical drainage,persistent exudation often occurs,which may lead to the formation of sinus tracts,fistulas,or ulcers.These complications contribute to delayed wound healing and can result in significant breast deformity.Furthermore,the disease is characterized by a prolonged clinical course and a high recurrence rate.According to traditional Chinese medicine(TCM),delayed wound healing after NPM rupture is primarily attributed to"deficiency of Qi(气)and blood,retention of pathogenic factors".In the later stages of wound healing following NPM rupture,chronic wounds often manifest as Yin(阴)syndrome wounds.The TCM external treatment method known as"accelerating pus secretion to promote tissue regeneration"has been traditionally used for such wounds.This approach improves the wound microenvironment,alleviates persistent inflammation,and facilitates tissue repair after NPM rupture,demonstrating favorable clinical outcomes.In terms of treatment,the commonly used drugs include the Shengji Yuhong ointment(生肌玉红膏)and the Huiyang Shengji ointment(回阳生肌膏),etc.From the perspective of modern medicine,Macrophage polarization plays a crucial role in wound healing.When induced by different microenvironment stimuli,its function and phenotype differentiate and activate into pro-inflammatory M1 macrophages and anti-inflammatory M2 macrophages with different functions.The mechanism of"accelerating pus secretion to promote tissue regeneration"method in promoting wound healing after NPM may be related to the orderly polarization of macrophage polarization to M2 phenotype.Based on this understanding,this article aims to explore the mechanism underlying the wound-healing effects of the"accelerating pus secretion to promote tissue regeneration"method from the perspective of macrophage phenotypic polarization,integrating modern pathological insights with TCM external therapies.This analysis not only provides novel therapeutic strategies and theoretical support for managing chronic wounds following NPM rupture,but also contributes to the development of standardized protocols for drug formulation and application based on this TCM theory,thereby promoting its more systematic and evidence-based clinical application.关键词
