中医药防治糖尿病脂毒性肾损伤的思路与策略
Thoughts and strategies of traditional Chinese medicine in preventing and treating diabetic lipotoxic renal injury
摘要
Abstract
Lipotoxic renal injury plays a key role in the pathogenesis of diabetic nephropathy,with lipid metabolism disorders lead-ing to structural and functional damage to the kidneys.Traditional Chinese medicine(TCM)holds that when excessive nutrition or dys-function in transport and metabolism occurs,pathological fat is formed,which accumulates and transforms into phlegm turbidity,blood stasis,and turbid toxins."Spleen dysfunction leads to internal generation of phlegm turbidity;liver fails in dispersion and dredg-ing,causing qi stagnation and blood stasis;turbid toxins and blood stasis block and disrupt the kidney collaterals",these mechanisms contribute to the occurrence and progression of diabetic lipotoxic renal injury.Therefore,treatment should focus on the core pathogen-esis of"dysfunction of Zangfu organs,endogenous turbid toxins and blood stasis,and organ damage",with the liver,spleen,and kid-ney as the central targets,emphasizing the"linkage of the three organs and coordinated metabolism",and adopting organ-specific therapy.The spleen serves as the hub,treating the spleen and eliminating turbidity,emphasizing transport and cleansing.The liver serves as the pivot,regulating the liver and promoting qi,emphasizing dredging and balance.The kidney serves as the foundation,tonifying the kidney and reinforcing deficiency,emphasizing nourishment and consolidation.Pathological products such as"turbidity,toxins,and stasis"are addressed by resolving stasis and dredging collaterals,emphasizing elimination and patency.Through invigorat-ing the spleen,regulating the liver,tonifying the kidney,and dredging collaterals,the dynamic balance of metabolism and excretion is restored,thereby delaying the progression of diabetic lipotoxic renal injury and improving patient prognosis.关键词
