柴胡桂枝干姜汤合枳术汤治疗2型糖尿病肝热脾虚证的疗效观察
Clinical efficacy of Chaihu Guizhi Ganjiang Decoction combined with Zhizhu Decoction in the treatment of type 2 diabetes mellitus with liver heat and spleen deficiency syndrome

Abstract
Objective To observe the clinical efficacy of Chaihu Guizhi Ganjiang Decoction combined with Zhizhu Decoction in the treatment of type 2 diabetes mellitus(T2DM)with liver heat and spleen deficiency syndrome.Methods Sixty-four patients with T2DM characterized by liver heat and spleen deficiency who attended the endocrinology outpatient clinic or ward of Yuquan Hospital,Tsinghua University(Tsinghua University Hospital of Integrated Traditional Chinese and Western Medicine)from October 2023 to August 2024 were enrolled and were randomly divided into a control group and an observation group,with 32 patients in each group,using the random number table method.Both groups received diabetes health education and oral treatment with metformin hydrochloride tablets and dapagliflozin tablets.The observation group additionally received oral Chaihu Guizhi Ganjiang Decoction combined with Zhizhu Decoction.Both groups were treated continuously for 8 weeks.Clinical efficacy,traditional Chinese medicine(TCM)syndrome scores,glucose metabolism indicators[fasting blood glucose(FBG),2-hour postprandial blood glucose(2 h PBG),glycated hemoglobin(HbA1c)],lipid metabolism indicators[total cholesterol(TC),triglycerides(TG),low-density lipoprotein cholesterol(LDL-C),high-density lipoprotein cholesterol(HDL-C)],fasting insulin(FINS),insulin resistance index(HOMA-IR),and pancreatic β-cell function index(HOMA-β)were compared before and after treatment.Adverse reactions during the treatment period were recorded.Results The total effective rate in the observation group was 93.75%,higher than 75%in the control group(P<0.05).After treatment,TCM syndrome scores in both groups were lower than those before treatment(P<0.05),and scores in the observation group were lower than those in the control group(P<0.05).Levels of FBG,2 hPBG,and HbA1c decreased in both groups after treatment(P<0.05),with greater reductions in the observation group(P<0.05).HOMA-IR decreased while HOMA-β increased in both groups after treatment(P<0.05).The observation group had lower HOMA-IR and higher HOMA-β values than the control group(P<0.05).Levels of TC,TG,and LDL-C decreased in both groups after treatment(P<0.05),with lower values in the observation group compared to the control group(P<0.05).No adverse reactions were observed in either group during the treatment period.Conclusion Oral administration of Chaihu Guizhi Ganjiang Decoction combined with Zhizhu Decoction can improve clinical symptoms,regulate glucose and lipid metabolism,reduce insulin resistance,and enhance pancreatic β-cell function in patients with T2DM with liver heat and spleen deficiency syndrome.关键词
