凉血五花汤联合他克莫司软膏治疗面部激素依赖性皮炎的疗效观察
Clinical efficacy of Liangxue Wuhua Decoction combined with tacrolimus ointment in treatment of facial corticosteroid-dependent dermatitis

Abstract
Objective To observe the efficacy and safety of Liangxue Wuhua Decoction in the treatment of facial corticosteroid-dependent dermatitis(FCDD)of the toxin-heat accumulation type.Methods A total of 70 patients with FCDD of the toxin-heat accumulation type who attended the Department of Dermatology,Beijing Hospital of Traditional Chinese Medicine,were enrolled and randomly divided by the envelope method into an observation group and a control group,with 35 cases in each.Both groups received health education.The control group was treated with oral cetirizine hydrochloride tablets,external application of 0.03%tacrolimus ointment,and cold compresses with Qingre Xiaozhong Lotion.The observation group received oral modified Liangxue Wuhua Decoction,cold compresses with Liangxue Wuhua facial mask,and topical application of 0.03%tacrolimus ointment.Both groups were treated continuously for 28 days.Before and after treatment,the two groups were compared in terms of the Flushing Assessment Tool(FAST)score,Investigator's Global Assessment(IGA)score,CBS cloud mirror multispectral facial skin imaging results,dermoscopic lesion scores(background color,vascular morphology,vascular arrangement),Dermatology Life Quality Index(DLQI)score,as well as clinical efficacy and safety.Results After treatment,FAST scores in both groups were lower than those before treatment(P<0.05),and the scores for tightness,dryness,and burning sensation in the observation group were lower than those in the control group(P<0.05).IGA scores in both groups were lower than those before treatment(P<0.05),and erythema,papule/pustule,and desquamation scores in the observation group were lower than those in the control group(P<0.05).Facial skin imaging results in both groups improved compared with those before treatment(P<0.05),but the difference between groups was not statistically significant(P>0.05).Dermoscopic lesion scores in both groups were lower than those before treatment(P<0.05).The observation group had lower scores for background color and vascular morphology than the control group(P<0.05),while there was no significant difference in vascular arrangement scores between groups(P>0.05).DLQI scores in both groups decreased compared with those before treatment(P<0.05),with no significant difference between groups(P>0.05).The total effective rate in the observation group was higher than that in the control group(χ2=5.983,P<0.05).The incidence of adverse reactions did not differ significantly between the two groups(P>0.05).No abnormalities in liver or kidney function,or in routine blood or urine tests,were observed in either group.Conclusion Oral administration of modified Liangxue Wuhua Decoction combined with Liangxue Wuhua facial mask can significantly relieve clinical symptoms,improve skin lesions,and enhance quality of life in patients with FCDD,with good safety and tolerability.关键词
